When SAGENT® was founded, in 2006, we were aware of alarming statistics about medication errors. For example, between 2003 and 2006, 25,5301 errors related to look-alike or sound-alike medications were reported to the Medication Errors Reporting Program
and MEDMARX (an adverse drug event database).
The Institute for Safe Medication Practices (ISMP) had produced a list showing hundreds of drug names that were easily confused because they look alike or sound alike or both, and typical
generic packaging did nothing to relieve the confusion. So it was no surprise to learn that:
